Job summary

Breaking Ground's Sutphin Residence in Jamaica, Queens, seeks a Clinical Coordinator, Permanent Housing to lead a team of Case Managers and Program Coordinator, ensuring Housing First and trauma-informed care are integrated into intake, assessment and service planning. The role coordinates with multiple providers to help residents remain housed and access benefits and health services.

Qualifications

• NYS Licensed Masters Social Worker (LMSW) required
• Minimum 3 years of experience with related populations; supervisory and administrative experience preferred
• Thorough clinical understanding of Housing First, Harm Reduction, Motivational Interviewing and trauma-informed care models
• Experience with crisis intervention, risk assessments and incident management
• Proficiency with Microsoft Office Suite (Word, Excel, Outlook, Teams); familiarity with databases and learning new programs as needed
• Valid NY or NJ driver’s license with a good driving record preferred
• Bilingual preferred

Responsibilities

• Supervise a team of Case Managers and Program Coordinator
• Provide regular, structured supervision meetings for direct reports
• Promote strategies for case management using Housing First, Harm Reduction, CPI, Motivational Interviewing and trauma-informed care
• Develop and implement individual service plans; document client contact and progress
• Interview and assess potential clients
• Provide clinical direction and leadership to the program and troubleshoot client engagement and challenging behaviors
• Oversee development and implementation of service plans; document progress
• Assess and evaluate client functioning
• Supervise and conduct case conferences
• Coordinate care delivery with multiple service providers, including outreach teams
• Support and guide staff in deescalating conflicts
• Perform other duties as assigned
