Job summary

Breaking Ground in New York City seeks a Clinical Coordinator to supervise a team of Case Managers and ensure use of Harm Reduction and Housing First strategies, delivering high-quality care in a regulatory-compliant environment. The role reports to the Assistant Director, Programs, Outreach, and oversees documentation and client progress.

Responsibilities include developing service plans, interviewing potential clients, coordinating care with outreach teams, and guiding staff to achieve housing

Qualifications

• NYS Licensed Masters Social Worker (LMSW) required.
• Thorough clinical understanding of Housing First and Motivational Interviewing.
• Experience with crisis intervention and risk assessments.
• Ability to coordinate across internal and external stakeholders.
• Proficiency with MS Office; willingness to learn new systems.
• Bilingual preferred.

Responsibilities

• Supervise a team of Case Managers and conduct regular supervision meetings.
• Promote Housing First, Harm Reduction and trauma-informed practices.
• Develop and implement individual service plans with thorough documentation.
• Interview and assess potential clients and coordinate care with providers.
• Provide clinical direction, troubleshoot and strategize engagement strategies.
• Monitor progress and oversee case conferences; support de-escalation of conflicts.
• Ensure compliance with regulatory and agency standards.
