Job summary

Victory Programs is seeking a Case Manager to coordinate direct case management for families at Chamblet and Revision Family Homes in Dorchester. You’ll report to the Program Director for weekly supervision and assignments, and help residents with housing, finances, vocation, and education.

Responsibilities include coordinating services, facilitating aftercare, and documenting on HMIS/ETO. Local travel and strong advocacy are essential in this role.

Qualifications

• Bachelor degree with 1-2 years’ experience preferred, or high school diploma with 3+ years of client advocacy.
• Experience advocating for clients with government assistance agencies, housing authorities, landlords or realtors.
• At least 25 years old with MA driver’s license and clean driving record.

Responsibilities

• Coordinate and deliver weekly case management services to families at Dorchester homes.
• Assist with housing search, referrals, and housing applications.
• Transport clients to housing, legal, and medical appointments as needed.
• Develop and monitor service plans and document in HMIS/ETO systems.
• Participate in supervision, trainings, and team collaboration.
