Job summary

Victory Programs Inc. is seeking a Low Threshold Housing Case Manager to coordinate case management for residents at Willows in Boston. You will support housing stabilization, eviction-prevention and access to services, reporting to the Program Director and collaborating with health and social service providers.

You’ll build a resource network for clients with HIV and chronic health issues, navigate housing and service systems, and document outcomes to improve housing stability and well-being.

Qualifications

• Bachelor’s degree or higher; 1–2 years’ housing case management experience preferred.
• Experience housing search, retention and coordination of support services.
• Experience advocating with gov't agencies, housing authorities or landlords.
• Strong communication skills and ability to work with diverse populations.

Responsibilities

• Coordinate and deliver case management services to residents at Willows.
• Identify housing needs and advocate for resources to stabilize housing.
• Provide housing search and eviction-prevention support and assist with applications.
• Document individualized support plans with measurable goals and timelines.
• Maintain accurate client files and participate in weekly meetings and supervision.
