Job summary

ST VINCENT de PAUL OF BALTIMORE INC is seeking a Case Manager to provide high quality program-based case management for families in our Homeless Services Division. You will help clients develop goals aimed at housing stability, monitor progress, and connect them with services to transition from shelter to permanent housing.

Responsibilities include creating Service Plans, maintaining case notes, and coordinating with internal and external partners to secure housing supports and benefits.

Qualifications

• Bachelor’s Degree with 2 years’ experience working with persons who are homeless, at-risk for homelessness or those with a low socioeconomic status.
• Associate’s Degree with at least 4 years’ experience working with persons who are homeless, at-risk for homelessness or those with a low socioeconomic status, will be considered.
• Willing and able to work a flexible schedule, weekends, and evenings; if applicable.
• Knowledge of and respect for confidentiality.
• Empathy to support vulnerable populations.
• Motivational to encourage clients to follow their service plan and engage in service delivery.
• Strong verbal and written communication skills to work collaboratively with internal and external stakeholders.
• Critical thinking and problem solving to determine the best service plan for each client after assessing clients, analyzing notes from health care and social workers.
• Administrative organization to manage several different cases at once.
• Computer literacy to maintain and manage case records.

Responsibilities

• Develops with each client or family a detailed Service Plan centered on goals that assist in maintaining stable housing.
• Ensures the successful completion of assigned goals for client or family documented within the Individual Service Plan (ISP).
• Provides individual life skills support and support to address barriers to success.
• Provides a minimum of two case management sessions each month to participants.
• Monitors and provides community linkages for participants in health, mental health, education and employment, housing, mainstream benefits, and parenting assistance.
• Gather relevant data via the identified organizational tools to monitor activities.
• Develop transitional plan 90 days prior to discharge of every client and family household.
