Job summary

Community Housing Innovations, Inc. (CHI) seeks a full-time Case Manager for the Commack location to promote independence and permanent housing for clients.

You will provide direct services, referrals, budgeting support, and advocacy while maintaining program compliance and client progress. The role requires a Bachelor’s in human services or related field, with MSW preferred, and strong communication skills; bilingual Spanish is a plus.

Qualifications

• Education: Bachelor’s degree in human services or related field; Master’s preferred.
• Two years’ experience in homelessness or housing is preferred.
• Excellent verbal and written communication; English proficiency required.

Responsibilities

• Adhere to program regulations, policies, and procedures to ensure quality services.
• Assist clients in developing an Independent Living Plan (ILP) and tracking progress.
• Provide information, referrals, counseling, crisis intervention and advocacy.
• Develop and maintain a referrals network; collaborate with DSS and service providers.
• Assist with direct services, budgets, resumes, ADL skills as appropriate.
• Conduct face-to-face meetings, inspections, and maintain housing unit compliance.
