Job summary

The Salvation Army is seeking a Case Manager to provide supportive, relational services to low-income clients aiming for goals and family sufficiency. You will document case status, track progress, and coordinate referrals to ensure program outcomes.

Responsibilities include developing case plans, maintaining client confidentiality, and using databases to manage records. A strong ethics background and Safe From Harm training are required.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field, or equivalent experience.
• 2+ years of applicant interview or case management experience.
• Experience working with program target population preferred.
• Must have and maintain Safe From Harm training, and keep current as needed.

Responsibilities

• Facilitates case management within program guidelines; develops rapport and case plans; tracks progress and coordinates referrals.
• Maintains relationships with community agencies to coordinate services and promote Salvation Army offerings.
• Creates case plans and helps participants achieve goals through agency resources and referrals.
• Uses electronic database to enter, retrieve, and manage case records per protocols.
