Qualifications

• Bachelor degree in Social Work from an accredited university, or related field.
• One year of case management experience.
• First Aid and CPR certification, or ability to obtain within 90 days.

Responsibilities

• Interview clients to assess level of need and develop action plans.
• Assess psychological, emotional, medical, financial, social, and spiritual status.
• Complete paperwork and submit for weekly review.
• Orient clients to program rules, rights and policies within 48 hours.
• Attend and facilitate weekly house meetings as needed.
• Maintain confidential client files and document progress daily.
• Provide transportation to appointments as needed and report data as directed.
• Stay informed on community resources to support referrals.
