Job summary

Phipps Houses and Affiliates is seeking a dedicated case manager to provide intake, education on services, and referrals to community providers, including mental health and preventive health screenings. Some home visits may be required to assess housing needs.

The role includes translating services via Phipps contractors, collecting referral information, coordinating with service agencies, and supporting residents to maintain stable housing and prevent eviction.

Qualifications

• Minimum of Diploma or GED is required.
• College degree in social work is a plus.
• Two to three years’ experience in social service delivery with seniors or people with disabilities.
• Demonstrated working knowledge of supportive services and other resources in the area served by the project.
• Demonstrated ability to advocate, organize, problem-solve and provide results for families.
• Experience with HUD multifamily housing and annual requirements.

Responsibilities

• Provide intake, education about services and referrals to community providers.
• Provide translation services through Phipps contractors.
• Collect service agency information for referrals.
• Sponsor informational events related to health care, job search and life skills.
• Facilitate referrals of Self-Help Groups within the community.
• Monitor ongoing service provision and maintain progress records.
• Set up volunteer support programs with local service organizations.
• Help residents build informal support networks.
• Assist with completing recertification documents and gather necessary documentation.
• Determine resident eligibility for programs to maintain stable housing.
• Provide services for formerly homeless families.
• Conduct home visits for mobility or housing-benefit assessments.
