Job summary

Center for Family Services in Philadelphia, PA is hiring a Housing Case Manager to provide on-site case management and coordinate social services for residents of an affordable housing community.

You will link residents to community resources, coordinate health, literacy, job readiness, and engagement programs, and support families with education and wellness activities. The role requires flexibility for occasional evenings or weekends.

Qualifications

• High School Diploma/GED required; Bachelor's degree in Social Work, Education, or related field preferred.
• At least two years of experience working with program population and supportive housing experience.

Responsibilities

• Facilitate services that will positively enhance the quality of life for affordable housing residents.
• Coordinate programs and social services for children and adults leveraging community resources and agencies.
• Lead on-site food distribution programs and ensure residents have access to food resources.
• Track case management progress, demographic data, and monthly program attendance using data systems.
• Publish monthly newsletters and calendars to residents' doors.
• Provide excellent customer service to the partner property management company and residents.
• Ensure resident compliance with housing inspections and budgets following financial procedures.
• Attend mandatory trainings and conferences as required; occasional evenings/weekends may be needed.
