Qualifications

Responsibilities

• Cultivate and maintain relationships with residents to connect them with services.
• Identify presentation sites for health education across child care, schools, community centers, and housing programs.
• Track data in Salesforce and document client participation, referrals, and follow-up.
• Design activities to recruit residents for opportunities available.
• Coordinate data collection and maintain a database showing integration with residents and partner agencies.
• Participate in health coalitions, advisory meetings, and learning communities.
