Job summary

Phipps Neighborhoods, the social service affiliate of Phipps Houses, seeks a Case Manager for the Courtlandt Community site in New York. You will provide intake assessments, counseling, and referrals to residents of a supportive housing facility.

Qualifications

• Bachelor’s Degree preferred in Social Work, Psychology, Sociology, Human Services or related field.
• Minimum associate’s degree or 60 college credits with two to five years experience.

Responsibilities

• Conduct comprehensive intake assessments for new residents and review policies.
• Develop treatment plans for residents.
• Maintain concise resident files and provide referrals.
• Input data in Salesforce and Maven databases.
• Attend trainings, meetings and prepare monthly reports.
