Job summary

Qualifications

Responsibilities

• Cultivate and maintain relationships with residents to connect them with services.
• Seek presentation sites for health education in schools, centers, and community settings.
• Track data and document referrals and outcomes in the database.
• Create and distribute program marketing materials and reports.
• Coordinate data collection and assist in program evaluation and reporting.
