Job summary

Housing Forward seeks a Housing Case Manager to coordinate direct case management and stabilization services for clients in the Supportive Housing Program. The role focuses on housing advocacy and linkage to help clients retain permanent housing.

Responsibilities include developing service plans, maintaining documentation, and connecting participants with medical and mental health services. Requires a bachelor’s degree; master’s preferred, and a valid driver’s license for transporting clients.

Qualifications

• Bachelor’s degree in social work or related field; Master’s degree preferred.
• Will consider a combination of education and work experience.

Responsibilities

• Provide case management services for program participants and help them obtain permanent housing by addressing barriers.
• Develop and monitor service plans, maintain required documentation and logs, and ensure timely reporting.
• Transport clients to housing, legal, and medical appointments as needed.
• Connect participants to medical, mental health, and other supportive services.
• Conduct home visits to support lease compliance and program rules; participate in care coordination and team meetings.
