RS EDEN is hiring a Housing Case Manager to support clients in permanent supportive housing. You will engage clients at housing sites and in the community to develop an Individual Support Plan and link to resources.

The role emphasizes teaching, coaching, and strengths-based problem solving to improve housing stability, safety, and wellbeing. A flexible schedule, including one evening per week, is required.

Formación

• Associate’s degree and 2+ years of experience or lived in human services.
• Bachelor’s degree in human services or related field required.

Responsabilidades

• Provide Person Centered Case Management, Client Skills groups and Community Supports.
• Develop and maintain knowledge of available community resources.
• Act as a role model with professionalism and boundaries in service interactions.
• Maintain electronic health records and timely billing for services.
• Safeguard PHI per HIPAA and agency standards.
• Flexible schedule including at least one evening per week.
