Job summary

Eden Housing is seeking an Experienced Case Manager under the supervision of the Associate Director, Supportive Housing to oversee resident services programming at properties serving high-needs populations and low-income households.

The role coordinates services with county providers and external agencies to bring robust programming, maintain HIPAA compliance, and support residents' independent living and personal goals.

Qualifications

• Education and experience in social services or related field
• Experience serving high-need/senior populations preferred
• Experience with multiple assessment tools and data interpretation

Responsibilities

• Deliver onsite programs and ensure alignment with standards and regulatory requirements
• Conduct outreach to residents via door-to-door, phone, and email
• Provide crisis intervention and follow-up per mandated reporting policies
• Support case management for residents with functional impairments
• Coordinate services with residents to promote independent living and personal goals
• Develop collaborative relationships with service providers and property management
