Qualifications

• MSW/MA in psychology or related field with 1 year relevant experience OR BA/BS with 3 years relevant experience
• Experience serving homeless populations or diverse populations
• Experience using assessment tools and interpreting information/data from assessments
• Experience coordinating services and collaborating with local service providers

Responsibilities

• Deliver onsite programs independently at assigned locations meeting internal standards and regulatory expectations
• Conduct door-to-door, phone, email outreach to raise awareness of services
• Provide crisis intervention and follow-up including mandated reporting as needed
• Support case management for residents with functional impairment and aid independent living skills
• Provide group and individual services and foster community building at properties
• Collaborate with residents to support independence and goals
• Educate residents about program options and advocate for needs
• Maintain HIPAA policies and procedures
