Job summary

Access Services is seeking a compassionate Housing Case Manager to empower individuals in Montgomery County, PA on their journey toward stability and wellness. The full-time, on-site role blends mental health and housing stability skills, coordinating housing moves, services, and resources for clients with housing needs.

The role requires a Bachelor’s degree in a related field or Certified Peer Specialist certification, plus completion of the Psychiatric Rehabilitation Practitioner orientation

Qualifications

• Bachelor’s degree in human services, social work, psychology or special education required.
• Certified Peer Specialist certification required.
• Certified Psychiatric Rehabilitation Practitioner certification and 12‑hour orientation required within 12 months of hire.

Responsibilities

• Meet with referred individuals with Housing Locator and assist with housing moves.
• Develop and update Housing Stability Plans every three months.
• Maintain move-in, recertification and ongoing documentation.
• Conduct monthly home visits and support during transitions.
• Assist with transportation for health appointments, job interviews and activities supporting stability.
• Identify and address barriers to housing stability with creative solutions.
• Connect individuals to social, health and benefit resources.
• Ensure compliance with HUD requirements and funding guidelines.
• Participate in county/committee/case reviews.
• Develop and update Individual Rehabilitation Plans (IRPs) for Mobile Psychiatric Rehabilitation.
• Collaborate to identify and pursue personal goals; support with a person-centered approach.
• Maintain communication with supervisors; engage in weekly supervision.
• Collaborate with external agencies; represent Access Services with competence.
• Assist in resolving crisis situations.
