Job summary

Eden Housing, Inc. is seeking a Case Manager to oversee resident services programming at high-need properties within Eden Housing’s portfolio. You will coordinate services for low-income households and collaborate with county service providers to bring robust programming to residents.

Under the supervision of the Associate Director, Supportive Housing, you will deliver onsite programs, conduct outreach, assist in crisis response, support independent living goals, and maintain HIPAA compliance.

Qualifications

• BA/BS in social work, psychology, counseling, or related field; master's preferred.
• 3 years’ experience serving the needs of senior populations and special needs.
• Experience with Intellectual Development Disabilities.
• Experience using assessment tools and interpreting data.
• Experience coordinating programs and services.
• Experience collaborating with local service providers.

Responsibilities

• Deliver onsite programs to meet internal standards and regulatory expectations.
• Conduct outreach to inform residents of services offered.
• Assist in crisis intervention and follow up; report mandated concerns.
• Support case management for residents with functional impairment.
• Provide group and one-on-one services and community-building activities.
• Collaborate with residents and ILS/SLS workers to meet needs.
• Educate residents about options and advocate for needs.
