Magnificat Houses, Inc. is seeking a Case Manager to support chronically homeless and disabled adults, promoting stability and self-sufficiency through individualized service plans, benefits coordination, and community partnership referrals.

You will conduct in-person and phone consultations, maintain HMIS data, organize participant files, and collaborate with on-site staff to move participants toward mainstream housing. The role requires evening/weekend availability and a compassionate approach.

Qualifikationen

• Bachelor’s degree in psychology, sociology, or related field.
• 1–2 years of relevant experience.
• Knowledge of chronically homeless population and HMIS preferred.
• Excellent listening and communication skills.
• Effective communication in writing and speech.
• Ability to work evenings and weekends as needed.
• Ability to work independently and in a team.
• Proficiency in Microsoft Office Suite.
• Passion for Magnificat Houses, Inc. mission.

Aufgaben

• Assist participants to maintain long-term housing and understand fair housing rights.
• Assist participants in understanding the landlord/tenant relationship and becoming good leaseholders.
• Maintain current information about consumers’ benefits to ensure subsidy accuracy.
• Develop and maintain a good working relationship with the on-site property manager.
• Assume responsibility for data management and reporting; enter data into HMIS.
• Maintain complete and accurate participant files with current case notes and financial documentation.
• Exhibit and maintain knowledge of community resources; make referrals to partners.
• Provide services through in-person and phone consultations; conduct home visits monthly.
• Obtain start-up items for new tenants.
• Develop ISP with each program participant on goals.
• Facilitate community partner referrals and follow-up on outcomes.
• Maintain accurate client files (hard copy and digital).
• Other duties as assigned.
