Responsibilities

• Conducts intake and develops an ISP addressing housing goals and income.
• Provides referrals and connections to resources to support ISP goals.
• Completes program assessments (e.g., VI-SPDAT).
• Evaluates strengths and challenges, conducts 90-day reviews with participants.
• Maintains documentation per PATH policies and HIPAA guidelines.
• Develops professional relationships with clients and partners to support housing goals.
• Provides crisis intervention and risk assessment to increase safety.
• Uses motivational interviewing and trauma-informed care in service delivery.
