Job summary

Care Resource Community Health Centers, Inc. is seeking a Housing Coordinator to serve as a liaison and advocate between homelessness services and medical care teams, helping clients secure housing and access medical and social services.

You will accompany clients to appointments, assess housing barriers, and document needs for eligibility and case planning. The role requires 2 years of related experience, knowledge of homeless resources, and ability to work with diverse populations.

Qualifications

• Two (2) years of related work experience is required.
• Knowledge of homeless community resources and community services is required.
• Experience to work with multicultural and diverse population.
• Experience in Rapid Re-Housing program, Permanent Supportive Housing and FQHC program.

Responsibilities

• Serve as liaison, coordinator and advocate between Homelessness, Housing case Management, Medical Care departments or other community medical or agency service providers to remove homelessness barriers.
• Accompany clients to Housing, medical, mental health, substance use disorder treatment or social service appointments to improve Housing Stability and medical care.
• Create rapport with clients to help progress on Housing issues and prevent homelessness.
• Interview prospective clients to determine homelessness needs and eligibility for medical and social services, enrolling them into available programs.
• Maintain an organized tracking system (HMIS) and follow up to progress client outcomes.
• Conduct initial intake and housing barrier assessments and document information for eligibility and case plans.
• Identify neighborhoods for housing searches and negotiate with landlords on behalf of clients.
• Build a pool of landlords and property managers willing to rent to program participants; conduct outreach, education and public relations.
• Coordinate, support and follow up on medical treatments.
• Maintain an average annual active caseload as assigned by supervisor.
• Assist in processing and submission of housing applications (subsidized and unsubsidized).
• Input client information via electronic data entry per guidance.
• Maintain knowledge of community services and apply to client needs.
• Support billing with documentation of services and budget activities across data systems.
