Job summary

Sea Mar Community Health Centers seeks a Housing Placement Specialist to coordinate housing for clients with chronic and behavioral health needs. You will collect documents, manage referrals, and monitor applications across housing programs.

You will work with homed-out teams to expedite placements, develop stability plans, and connect clients to Sea Mar and community resources while maintaining confidentiality and compassionate, client-centered care.

Qualifications

• Excellent organizational skills and ability to handle a variety of tasks simultaneously.
• Knowledge of medical terminology and/or behavioral health topics.
• Strong decision making and prioritization skills.
• Ability to work respectfully and professionally with the community, patients, families and staff.
• Able to work effectively in a multi-cultural environment with a diverse population.
• Sympathetic, mature, responsible, and reliable.
• Strong client engagement, interpersonal, and communication skills and ability to establish a therapeutic relationship with the client.

Responsibilities

• Coordinate housing placement for clients with chronic conditions and behavioral health needs.
• Maintain lists, subsidies, vouchers and assist in housing applications.
• Monitor progress and update teams on status to expedite housing placement.
• Develop housing stability plans addressing barriers such as criminal history and evictions.
• Coordinate with housing providers and landlords; facilitate communications.
• Work with care team to identify gaps and resolve via process improvements.
• Provide education and referrals to benefits and community resources.
• Track program data, reporting and paperwork.
• Advocate for client services with housing, social services and medical providers.
• Support care plans for clients without housing and connect to Sea Mar resources.

Education

BA/BS/BSW in health-related field

Peer Support Certification

Basic Life Support (BLS) CPR within 90 days

Microsoft Word/Excel/PowerPoint/Outlook
