Job summary

Pacific Clinics is seeking a Cal-AIM Housing Navigator to ensure timely and quality housing navigation services, deposits, and tenancy support across the county. You will collaborate with CoC homeless services, MCPs, and ECM providers to deliver housing supports to members.

The role requires housing/homeless service experience and HUD funding knowledge; travel/coordination with community partners is expected.

Qualifications

• High School Diploma or GED with 4+ years of relevant experience.
• Knowledge of housing rights pertaining to homelessness and HUD funding.
• Experience with Motivational Interviewing, Trauma-Informed Care, Harm Reduction.

Responsibilities

• Develop outreach strategies for consumers who tend to isolate themselves.
• Demonstrate commitment to consumer choice and empowerment.
• Assist clients with housing applications, deposits and linking to housing resources.
• Maintain HMIS and Salesforce documentation as required.
