Care Resource Community Health Centers, Inc. is seeking a HOPWA and Housing Opportunities Coordinator to screen and assess new clients, complete program applications, determine eligibility, and refer applicants to appropriate housing resources.

You will manage caseloads, document progress, coordinate with internal and community partners, and support billing and reporting. A bachelor’s in social work or related field and at least two years of homeless services experience preferred.

Qualificações

• Bachelor's degree in Social Work or related field.
• A minimum of 2 years of experience in homeless or social services.
• Ability to screen, assess, and refer clients to housing services.

Responsabilidades

• Screen and assess new clients for HOPWA services and other housing stability services according to intake schedule.
• Complete applications for HOPWA programs and other housing programs.
• Determine eligibility for HOPWA programs and other housing programs.
• Identify resources and refer applicants to appropriate housing programs.
• Follow up with clients on housing stability (HOPWA and non-HOPWA housing).
• Refer clients to Rapid Rehousing program.
• Maintain progress notes and progress reviews in client records as specified in agency policy.
