Concierge Home Care in Brevard County, FL is seeking a Hospital Liaison (RN or LPN) to bridge hospital teams, physicians, patients, and home health services during transitional care. You will build referral relationships and coordinate post-discharge care to drive admissions growth within the assigned hospital systems.

Qualifications

• Active Florida RN or LPN license.
• Strong communication and interpersonal skills.
• Detail-oriented and organized.
• Valid driver’s license and reliable transportation.

Responsibilities

• Build relationships with case managers and hospital staff.
• Meet patients bedside prior to discharge and explain home health benefits.
• Coordinate care from hospital to home with care teams.
• Process referrals in real time and complete admission docs.
