Key Responsibilities

Hospital & Referral Relationships

Build relationships with:

Patient & Family Support

• Meet patients bedside prior to discharge
• Explain home health services and benefits

Transitional Care Coordination

• Coordinate care from hospital to home
• Collaborate with:

• DME providers
• PCPs
• Caregivers

• Partner closely with:

• Intake teams

• Provide timely referral and patient updates
• Ensure continuity of care throughout admission process

Referral & Documentation Management

• Process referrals in real time
• Complete admission documentation accurately and timely

Qualifications

Required

• Active Florida RN or LPN license
• Strong communication and interpersonal skills
• Detail-oriented and organized
• Valid driver’s license and reliable transportation

• Home health experience
• Hospital liaison or healthcare marketing experience
• Case management or transitional care experience
• Knowledge of payer sources and referral processes
