Job summary

Concierge Home Care in Davenport, FL is seeking a motivated Home Health Account Executive to develop referral partnerships, increase patient admissions, and serve as the trusted resource for physicians, hospitals, SNFs and other providers.

This is a full-time position offering base salary of $70,000-$90,000 plus uncapped bonuses, with benefits and autonomy to grow your territory in Polk County and surrounding areas.

Qualifications

• Healthcare sales experience or strong clinical relationships.
• Excellent communication and relationship-building skills.
• Reliable transportation and a valid driver's license.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, case managers, and other referral sources.
• Increase home health admissions and achieve monthly sales goals.
• Execute strategic territory management plans.
• Educate referral partners on Concierge Home Care’s services.
• Coordinate smooth patient transitions from hospital or facility to home.
• Serve as a trusted healthcare resource within the community.
• Collaborate closely with clinical leadership to ensure outstanding patient outcomes.
• Maintain CRM documentation and referral activity.
• Represent Concierge Home Care at networking events and community outreach opportunities.
