Job summary

Genie Matthews and Associates is seeking a Hospital Liaison in the New York area to drive post-acute intake and census growth across Brooklyn and Queens within major NYC health systems.

This full-time, on-site role blends clinical liaison with admissions leadership. You will build hospital relationships, perform pre-admission evaluations, coordinate transfers, and collaborate with nursing, therapy, and admissions teams to optimize referrals and patient transitions.

Qualifications

• Minimum of 2+ years of direct SNF/LTC experience in a liaison/admissions role.
• Proven hospital relationships with case management teams in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field preferred; active clinical credential is a strong plus.
• Knowledge of acute hospital discharge workflows, SNF/LTC admission criteria, Medicare Part A, Medicaid, and pre-authorizations.

Responsibilities

• Maintain and cultivate relationships with hospital staff across Brooklyn and Queens.
• Perform pre-admission evaluations and chart reviews to determine patient suitability.
• Convert clinical leads into admissions to exceed census targets.
• Advise patients and families on sub-acute rehab programs and transitions.
• Collaborate with admissions teams, nurse directors, and therapy leaders for care plans and pre-authorizations.
• Monitor local healthcare trends and educate hospital case management teams.
