Job summary

LifeCare Home Health & Hospice Family in Scottsdale, AZ is seeking a Patient Care Advocate to drive overall development and referral growth for the assigned territory, including facilities, physicians, care-at-home providers, and community agencies. You will build strong relationships with referral sources and internal stakeholders to promote hospice programs and services.

The role requires a Bachelor’s degree or equivalent experience, at least two years in hospice sales or related fields, and

Qualifications

• Bachelor’s degree in marketing or related field or equivalent professional experience.
• Minimum of two (2) years of experience in hospice sales or a related industry with a proven record of achieving incremental growth.
• Excellent oral communication skills and the ability to work with little supervision.
• Proficiency in using Microsoft programs for communications, presentations, data tracking, and reporting.
• CRM experience preferred and demonstrated ability to establish professional relationships.

Responsibilities

• Responsible for the overall development and referral growth of assigned territory, including facilities, physicians, care-at-home providers, and community agencies.
• Fosters good working relations with physicians, third-party payors, and community agencies.
• Develops, implements, and evaluates territory plans to achieve growth goals and implement strategies.
• Develops business relationships with providers through effective sales calls and referrals.
• Serves as liaison between referral partners and agencies to communicate information about services.
• Meets with patients and families to discuss home care services and manage expectations.
• Builds relationships within the Intake department and with patient care resources staff to ensure effective communication with referral sources.
• Updates and maintains customer and competitor profiles, logs daily call activity, and documents follow-up plans.
