Job summary

Lifecare-Home-Health-Family-1 is seeking a Patient Care Advocate to drive growth in the Henderson, NV area by developing partnerships with physicians, facilities, and community agencies. You will present hospice programs, manage referrals, and coordinate with intake and clinical teams to ensure quality end-of-life care.

The role requires building relationships, meeting performance goals, and traveling locally to referral sources.

Qualifications

• Bachelor's degree in marketing or related field or equivalent professional experience.
• Minimum of two years of hospice sales or related industry with growth in direct sales.
• Proficient in Microsoft programs for communications, presentations, tracking, and spreadsheets.
• CRM experience preferred; excellent oral communication; self-directed with minimal supervision.

Responsibilities

• Develops territory growth including facilities, physicians, care-at-home providers, and community agencies.
• Builds relationships with physicians, payors, and referral sources.
• Creates and evaluates territory plans to meet growth goals.
• Communicates with referral sources and internal teams to ensure service delivery.
• Educates healthcare providers about hospice programs and services.
