Infinity Hospice Care LLC is seeking a Patient Care Advocate to develop and promote hospice programs in Scottsdale and surrounding areas. You will grow referrals, make effective sales contacts, and deliver presentations while upholding high standards of quality end‑of‑life care.

The role builds relationships with physicians, discharge planners, facilities, payors, and community agencies, coordinating with intake staff to ensure clear communication.

Qualifikationen

• Bachelor’s degree in marketing or related field or equivalent professional experience.
• Minimum of two years of hospice sales or related industry with a proven growth record.
• Proficiency with Microsoft Office for communications and reporting.
• CRM experience is preferred.
• Excellent oral communication skills and self-direction.

• Develops and grows referral networks within the assigned territory.
• Fosters relationships with physicians, payors, and community agencies.
• Develops and implements quarterly and annual territory plans to achieve growth goals.
• Delivers sales calls and presentations to targeted referral sources.
• Collaborates with Intake and care teams to ensure effective communication.
• Maintains provider and competitor profiles and documents follow-up plans.
