Affinity Hospice Management LLC is seeking a dedicated hospice aide for a night shift (5pm - 10pm) in Parma, OH. The role supports RN supervision and delivers personal care to promote comfort per the patient’s plan of care.

Responsibilities include communication with patients and families, safe patient handling, bathing, dressing, feeding support, observation, and documentation in EMR. Prior hospice/home health experience is preferred.

Formación

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification.
• Covid vaccination as per facility guidelines due to multiple facilities and covid protocols.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
• Prior hospice/home health/acute or long-term care experience preferred (1 year).
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsabilidades

• Provide personal care tasks and assist with activities of daily living.
• Communicate effectively with patients and families and maintain confidentiality.
• Ensure patient safety and use equipment properly.
• Assist with baths, grooming, oral care, hair care and linen changes.
• Assist in dressing, feeding and mobility as assigned.
• Support with light housekeeping and laundry tasks.
• Monitor skin condition and report changes to RN.
