Responsibilities

• Performing personal care needs and attending to patient requests promptly.
• Providing patient and family with respectful communication and maintaining confidentiality.
• Ensuring safety and proper use of equipment.
• Giving personal care including baths, oral hygiene, shampooing, and changing bed linens.
• Assisting in dressing and undressing.
• Assisting in feeding as assigned and noting dietary restrictions.
• Monitoring skin to prevent tissue breakdown.
• Reporting changes to RN Case Manager.
• Assisting in ambulation and exercises as instructed.
• Providing range of motion and transfers as directed.
• Assisting with self-administered medications as allowed by state regulations.
• Assisting with bedpans, urinals, and light housekeeping duties.
• Performing routine procedures with competency as ordered.
• Providing respite for patient/family when on-site.
• Keeping living area clean and orderly.
• Adhering to documentation and care procedures.
• Participating in quality improvement teams.
• Respecting patient and family environment and needs.
• Reporting any compliance issues to authorities as required.
• Recordkeeping and communication about patient status as per policy.
