Job summary

Affinity Hospice Management LLC is seeking a compassionate hospice aide to work under the supervision of a registered nurse and implement the patient’s plan of care. You will perform personal care tasks, promote comfort, monitor safety, and report observations promptly to the Case Manager to ensure quality and continuity of care.

Core duties include assisting with bathing, dressing, feeding, mobility, and routine housekeeping, providing respectful communication to patients and families, and

Qualifications

• Completion of a state-approved CNA course.
• Current Healthcare Provider CPR certification.
• Covid vaccination as a condition of employment.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and terminally ill care.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care tasks and attend to patient requests.
• Provide respectful communication with patients and families.
• Meet safety needs and use equipment properly.
• Assist with bathing, dressing, feeding, and bed linen changes.
• Assist in dressing and transferring patients as assigned.
• Report observations and changes to the RN Case Manager.
• Assist in mobility and range of motion as directed.
• Maintain patient area cleanliness and orderly environment.
