Qualifications

• Requires completion of a state-approved CNA program or equivalent training.
• High school diploma or GED required.
• CPR certification required; hands-on skills demonstration preferred.
• COVID-19 vaccination as per facility policy.
• Ability to read and follow written instructions and document care.
• Basic computer skills for EMR documentation.

Responsibilities

• Provide personal care including baths, grooming, and basic hygiene.
• Assist in dressing, feeding, and mobility while promoting safety.
• Report pain, changes in condition, and observations to RN Case Manager.
• Maintain cleanliness and comfort of patient living area and supplies.
• Communicate effectively with patients, families, and staff with sensitivity and confidentiality.
• Adhere to care plans and organizational documentation standards.
• Support quality assessment and safety initiatives as needed.
