Job summary

Affinity Health Management is seeking a compassionate Hospice Aide to support patients under an RN's supervision, providing personal care and comfort aligned with each plan of care. Responsibilities include reporting observations, ensuring safety, assisting with activities of daily living, and maintaining patient dignity and confidentiality in a home or facility setting.

Qualifications

• Completion of state-approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Covid vaccination as per facility/covid guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Hospice philosophy understanding; comfort with terminally ill patients.
• Prior hospice, orderly, acute, rehabilitative, or long-term care experience preferred (1 year).
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks after patient assignment and respond promptly to requests.
• Provide positive communication with patient and family, maintaining confidentiality.
• Ensure safety and proper use of equipment.
• Give personal care (baths, back rubs, oral care, shampoos, linen changes).
• Assist in dressing and undressing patients as assigned.
• Assist in feeding; avoid homemade food unless approved.
• Monitor skin condition to prevent pressure injuries.
• Report pain or changes to RN Case Manager.
• Assist in ambulation and therapeutic exercises as directed.
• Provide ROM and positioning assistance.
• Assist with self-administered medications as allowed by state rules.
• Offer bedpans/urinals and light housekeeping tasks.
• Perform ROM and simple procedures with demonstrated skills.
• Provide respite for patient/family/caregiver when on-site.
• Keep living area clean and orderly.
• Follow care procedures and professional conduct standards.
• Participate in quality improvement activities.
• Respect patient and family environment and needs.
• Report concerns per regulations to appropriate health departments.
