Job summary

Affinity Hospice Management LLC is seeking a hospice aide to perform personal care, monitor patient needs, and document observations under RN supervision. The role emphasizes safety, compassionate communication, and adherence to care plans.

Responsibilities include assisting with daily activities, mobility, and basic care tasks while maintaining confidentiality and professional standards across multiple facilities and patient settings.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on, not internet-based unless observed).
• Covid vaccination required due to work with multiple facilities and guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable providing specialized end-of-life care.
• Prior hospice/home health/long-term care experience preferred; 1 year experience.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care after assignment and attend to requests promptly.
• Provide patient and family with positive communication techniques, including confidentiality.
• Meet safety needs and use equipment safely (foot stools, side rails, O2).
• Provide personal care including baths, back rubs, oral hygiene, shampoos and changing linens.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding the patient as assigned; no homemade foods by the aide.
• Provide proper skin care and monitor for tissue breakdown.
• Report pain or changes to RN Case Manager; aware of caregiver dynamics.
• Assist in ambulation and exercise as instructed by nurse or therapist.
• Provide ROM and transfers/positioning as directed.
• Assist with self-administered medications as allowed by state regulations (reminders).
• Offer bedpans and urinals; light laundry and homemaker tasks.
• Perform ROM and simple procedures as ordered with competency.
• Provide respite for patient/family/caregiver when on-site.
• Keep living area clean and orderly.
• Adhere to documentation and care procedures and conduct standards.
• Participate in quality assessment and performance improvement teams.
• Respect patient and family environment and needs.
• As mandatory reporter, report noncompliance with health regulations per RCW within 14 days.
• Report suspected abuse/neglect to proper authorities immediately.
