Job summary

Affinity Hospice Management LLC is seeking a hospice aide who will work under RN supervision to meet a patient's personal care needs and comfort goals. The role includes reporting observations, ensuring safety, and supporting family caregivers.

Key duties cover personal care, mobility support, hygiene, feeding assistance, and light housekeeping, with strict adherence to care plans and documentation standards. Experience in hospice or home health is preferred.

Qualifications

• Completion of state approved CNA course or equivalent training.
• CPR certification (in-person skills demonstration preferred).
• COVID vaccination per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Comfortable with hospice philosophy and terminally ill care.
• Prior home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age; valid driver with insured vehicle.
• Knowledge of patient safety and equipment use.

Responsibilities

• Perform personal care per patient assignment and respond promptly.
• Communicate with patient and family respectfully, maintaining confidentiality.
• Ensure safety and use equipment properly (foot stools, side rails, O2).
• Provide baths, back rubs, oral hygiene, hygiene, and change bed linen.
• Assist with dressing/undressing and feeding per plan.
• Avoid providing homemade foods; follow dietary guidelines.
• Observe skin, prevent tissue breakdown and report changes to RN.
• Support ambulation and ROM as directed by nurse/therapist.
• Assist with medications reminders as allowed by state rules.
• Assist with bedpan/urinal, light laundry, and housekeeping tasks.
• Operate within scope; document care and follow procedures.
• Provide respite by coordinating with caregiver and team as needed.
• Maintain a clean living area and comply with professional standards.
