Job summary

Affinity Hospice Management LLC is seeking a compassionate hospice aide to work under the supervision of a registered nurse, assisting with personal care, comfort, and daily activities. You will report observations and care performed to ensure quality, continuity and safety of patient care.

The role involves assisting with bathing, dressing, feeding, mobility, and basic documentation in EMR, while upholding confidentiality and patient dignity. A CNA course, CPR, and vaccination are required.

Qualifications

Completion of a state-approved CNA course or equivalent.
Current Healthcare Provider CPR certification with hands-on demonstration.
Covid vaccination due to multiple facility work.
High school diploma or GED.
Ability to read and follow written instructions and document care.
Understands hospice philosophy and cares for the terminally ill.
1 year of home health/hospice experience preferred.
Basic computer skills to document in EMR.
Must be at least 18 years old.
Licensed driver with insured vehicle in good working order.

Responsibilities

Perform personal care and tasks as assigned by the RN Case Manager.
Provide compassionate communication with patient and family, maintaining confidentiality.
Ensure safety and proper use of equipment (e.g., O2, rails, etc.).
Assist with bathing, grooming, and linen changes.
Assist in dressing/undressing and feeding as directed.
Avoid giving homemade foods from caregivers; follow policy.
Monitor skin condition to prevent breakdown and report changes.
Report pain or changes to the RN Case Manager promptly.
Assist in ambulation, transfers, and range of motion as instructed.
Provide reminders for self-administered medications within state rules.
Assist with bedpans/urinals and light housekeeping/laundry.
Complete procedures as competency is demonstrated and documented.
Offer respite to patient/family when on-site.
Maintain a clean, orderly living area.
Adhere to care procedures and professional conduct.
Participate in quality assessment and improvement activities.
Respect patient/family preferences and environment.
Report safety or regulatory concerns as required.
