Job summary

Affinity Health Management is seeking a Hospice Aide to support patients under the supervision of a registered nurse. The role focuses on personal care, comfort, and accurate reporting of observations per the patient’s plan of care.

The aide will be assigned to ensure quality, continuity, and safety of patient care, including assisting with activities of daily living and light homemaker tasks, while adhering to confidentiality and organizational standards.

Qualifications

• Completion of a state approved certified nursing assistant course.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Covid vaccination as per job requirements.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable caring for terminally ill.
• Prior hospice/long-term care experience preferred (1 year).
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle per organization requirements.

Responsibilities

• Perform personal care tasks and respond promptly to patient requests.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and proper use of equipment during care.
• Provide baths, grooming, oral hygiene, and bed linen changes.
• Assist with dressing, undressing, and feeding as assigned.
• Avoid giving homemade foods; follow patient plan of care.
• Monitor skin for breakdown and report concerns to RN Case Manager.
• Assist in ambulation and transfers as directed by the nurse/therapist.
• Support self-administered medications as allowed by state regulations.
• Maintain clean and orderly living area and assist with light homemaker tasks.
• Document observations and care in EMR as required.
