Qualifications

• Completion of a state approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification.
• Covid vaccination as required by facilities.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and terminally ill care.
• 1 year of home health or hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with a valid insured vehicle.

Responsibilities

• Provide personal care such as baths and grooming as assigned.
• Assist in dressing, transferring, and ambulation.
• Observe and report patient condition changes to the RN.
• Offer respectful communication with patients and families.
• Maintain safety and proper use of equipment.
• Assist with feeding as permitted by state regulations.
• Maintain patient environment and light housekeeping.
• Adhere to documentation standards and care procedures.
