Job summary

Affinity Hospice Management LLC is seeking a compassionate hospice aide to support patient care under the supervision of an RN. You will perform assigned personal care, maintain comfort, and document observes care delivered while following care plans and safety standards.

The role emphasizes communication with patients and families, safe equipment use, and timely reporting of changes to the nurse in charge. Employment is full-time in a care setting, with adherence to hospice philosophy.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification.
• COVID-19 vaccination as required by facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Hospice philosophy understanding and terminally ill care familiarity.
• Prior home health/hospice experience preferred.
• Basic computer skills to document electronically (EMR).
• At least 18 years of age.
• Licensed driver with a compliant vehicle.

Responsibilities

• Perform personal care needs as assigned and report gaps promptly.
• Provide positive communication with patient and family, within ethical care.
• Meet safety needs and use equipment safely (foot stools, side rails, O2).
• Assist with baths, back rubs, oral hygiene, shampoos, bed linens changes.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding; avoid homemade foods prepared by staff.
• Observe skin integrity and report tissue issues.
• Report pain or changes to RN Case Manager.
• Assist in ambulation and prescribed exercises.
• Provide ROM and positioning as directed.
• Assist with self-administered medications as allowed by state rules.
• Offer bedpans/urinals and light homemaker tasks.
• Perform range of motion and simple procedures with competency.
• Provide respite for patient/family when on-site.
• Keep living area clean and orderly; follow procedures and conduct.
• Participate in quality improvement teams and activities.
• Respect patient/family environment and needs.
• As mandated reporter, report regulatory non-compliance within required timelines.
• Report abuse, neglect, or exploitation to authorities as required.
