Job summary

Compassus is seeking a hospice-trained Chaplain to provide spiritual support to patients and families. The role involves explaining hospice services, coordinating with the IDT, and delivering compassionate, nondiscriminatory care according to patient beliefs.

The position emphasizes grief and bereavement support, collaboration with clergy, and maintaining comprehensive care plans. A background in pastoral care and experience with end-of-life matters are essential.

Qualifications

• High school diploma or GED required.
• Bachelor's degree in Divinity, Social Sciences, or related field preferred, unless state regulation requires otherwise.
• Completion of at least one unit of CPE preferred, unless required by state regulation.
• Minimum of one year of experience providing spiritual/pastoral support.

Responsibilities

• Explain hospice services and Medicare benefits; obtain consent and election of benefits documents.
• Submit documentation to facilities upon patient visits; ensure timely submission.
• Participate in IDT; develop and implement care plans reflecting patient goals.
• Provide spiritual care to patient and family; support involvement of community clergy upon request.
• Collaborate with IDT to ensure continuity and quality of care; coordinate spiritual care.
