Job summary

VIA Health Partners seeks a Chaplain to provide spiritual care for patients and families in a hospice setting. The role includes offering prayers, sacraments, and grief support, while collaborating with physicians, facility staff, and the care team.

You will develop and update care plans, coordinate with community spiritual resources, and participate in IDT meetings to ensure compassionate, patient-centered care.

Qualifications

• Master’s Degree in Divinity from an accredited seminary and at least two units of Basic Clinical Pastoral Education required.
• Clinical experience in the area of death and dying, preferably in hospice care.
• Minimum of two years related experience; ordination or endorsement by a recognized religious body required; three years preferred.
• Equivalent combination of education, training, and work experience considered.
• Basic computer skills including Microsoft Office.

Responsibilities

• Provide spiritual care for patients and caregivers, respecting diverse beliefs.
• Offer prayers, sacraments, religious rites, scripture, and funeral planning as requested.
• Develop, implement, and evaluate care plans for patients and caregivers.
• Coordinate with community spiritual care resources and interdisciplinary teams.
• Provide grief care information and participate in IDT and case conferences.
• Support anticipatory grief interventions and continuation of care after death.

Education

Master’s Degree in Divinity

Basic Clinical Pastoral Education (CPE) – 2 units

Ordination or formal endorsement by a religious body

Clinical experience in death and dying
