CommuniCare Family of Companies seeks a Hospice Chaplain to provide spiritual support to patients and families in end-of-life care. You will implement and coordinate spiritual activities within the patient care program and collaborate with the care team to honor diverse belief systems.

The role emphasizes empathetic engagement, bereavement follow-up, and building a network of clergy for delegated care, ensuring compassionate, person-centered service in our southeast Ohio communities.

Qualifications

• Education in theology or pastoral ministry is preferred.
• Experience with patients and families facing life-limiting illness preferred.
• Licensure or certification as required by the state.

Responsabilités

• Provide direct spiritual support to patients and families.
• Collaborate with team members and clergy to address spiritual needs.
• Attend conferences/meetings to assess patient spiritual needs.
• Offer bereavement follow-up services as indicated by care plans.
• Develop relationships with reverends for delegated spiritual care.
• Prepare accurate patient documentation and submissions.
