Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families, coordinating hospice care within an interdisciplinary team. The role emphasizes assessments, bereavement planning, and culturally sensitive support.

Qualifications

• Masters in Social Work required.
• Hospice experience of 1+ year is required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) licensure (IL) is required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for families.
• Educate patients and families on cultural and spiritual considerations and resources.
• Complete visit notes and orders per policy and update care plans.
• Document MSW interventions in the electronic medical record.
• Collaborate with IDT to develop and adjust social work plans of care.
