Qualifications

Responsibilities

• Perform psychosocial assessments for patients and families.
• Develop bereavement plans to support relatives and caregivers.
• Educate patients and families respecting beliefs and learning style.
• Document assessments and care plans in the EMR.
• Collaborate with interdisciplinary teams to coordinate care.
• Identify and address barriers to community resources and benefits.
