Job summary

Carle Health seeks a Home Services Social Worker to identify psychosocial needs of patients and families and coordinate resources through a multidisciplinary team. The role involves short-term counseling, crisis intervention, and development of care plans aligned with patients' goals and Hospice guidance.

The ideal candidate holds a Master’s in Social Work, is licensed or licensed eligible (LSW/LCSW), and has Hospice experience of 1+ year. A sign-on bonus is available.

Qualifications

• Master's degree in Social Work required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) required.
• Driver's license and proof of auto insurance required.
• Hospice experience of 1+ years is required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care addressing needs of family/caregiver.
• Communicate patient/family faith and culture considerations to the team.
• Educate patients and families and address barriers to learning.
• Document assessments and interventions in the electronic medical record.
• Implement and adjust social work plan to maximize patient and family coping.
