Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop a bereavement plan of care for family/caregivers.
• Educate patients and families respecting beliefs and learning needs.
• Document assessments and interventions in the electronic medical record.
• Support patient decisions and collaborate with the IDT.
